Abstract
Objective To investigate the postural care on patients with unilateral pleural effusion of blood oxygen saturation and complications. Methods Sixty-four patients with unilateral pleural effusion were recruited and assigned to two groups according to fluid volume, small amount of fluid group (A group, n=34) and massive fluid group (B group, n=30). In the quiet situation, patients were taken contralateral supine and ipsilateral supine for 5 minutes in a state without oxygen and suffering. Chest distress, symptoms of chest pain, coughing, dyspnea were observed and recorded. Then the affected side, contralateral position was held in 15 min, respectively. Heart rate at different postures, breathing, blood oxygen saturation, and complications were recorded with EDAN multi-parameter monitor. Results There were statistical differences in respiration rate, heart rate, oxygen saturation and complications (chest distress, chest pain, cough and dyspnea) when taking healthy lateral decubitus. When taking the affected side decubitus, the heart rate, oxygen saturation value, chest distress, chest pain, cough were significantly different (P<0.05), while the dyspnea and respiratory rate were not statistically different. Conclusion The patients with medium or low level of pleural effusion were advised to take Andral’s decubitus, while those who with high level were advised to take affected side decubitus. When decubitus change causes oxygen saturation decline, analyze and turn to proper decubitus.关键词
