以网络平台为媒介的糖尿病三级管理团队的应用研究

摘要
Abstract
Objective:To evaluate the effect of ongoing management of diabetes three level management team via network platform on diabetes patients.Methods:A total of 84 cases of diabetes mellitus patients who met the inclusion standard and admitted into department of endocrinology in Gansu provincial hospital from January 10 to February 10 in 2015 were divided into experimental group and control group,42 cases in each.Both groups of patients were evaluated by the specialist doctors on their diabetes diet and self management skills.After dis-charging,on the diabetes comprehensive information management platform,the experimental group of patients were allocated based on their communities,at the same time the patients’examination and inspection reports and assessment results were delivered and to train the community personnel;community personnel received the network intervention based on the assessment results of general hospitals;patients carried out the self directed learning on the comprehensive information management platform,and realized the interaction with community and general hospitals,lasting for nine months.The change values of type and amount of diet,self management skills,fasting plasma glucose and glycated hemoglobin were collected after nine months in both groups,and then to compare the difference between both groups.Results:After intervention,the type and amount of diet in exper-imental group of patients were closed to China Nutrition Association recommended dietary standards,and the self management skills were higher than that in control group(P <0.05);the values of fasting plasma glucose and glycated hemoglobin in experimental group were lower than that in control group(P <0.05).Conclusion:Di-abetes three level management team via network platform could enhance the self management skills of diabe-tes patients and promote them to adopt healthy diet habits,which had realized the integration management mod-el of general hospitals,community and patients.关键词
