血清HE4水平对预测卵巢癌铂类化疗效果的临床观察
The clinical study of HE4 in predicting the response of platinum-based chemotherapy in ovarian cancer

Abstract
Objective:To assess the role of HE4 in predicting platinum response during first-line chemotherapy in ovarian cancer. Methods:The concentrations of serum HE4 and CA125 were determined by electrochemiluminescence( ECLIA) in 52 patients with epithelial o-varian cancer before treatment and 6 months after the end of platinum-based first-line chemo-therapy ( follow-up) . The correlation of HE4 and CA125 expressions with the platinum-based chemotherapy response were analyzed. Results:52 patients included,31 were platinum sensitive and 21 were platinum resistant in terms of response to platinum-based first-line chemotherapy. In platinum resistance group,the levels of serum HE4 and CA125 before treatment were signifi-cantly higher than that of platinum sensitive group. Serum HE4 before treatment for predicting chemotherapy efficacy performed better than CA125 . Median HE4 concentrations at follow-up differed between platinum resistant and platinum sensitive ( 276. 5 vs 56. 38pmol/L, P<0. 001),the HE4 value of 127. 40pmo/L at follow-up was the best cut-off to predict platinum response with a sensitivity of 81% and a specificity of 93 . 5% ( PPV=89 . 5% and NPV=87. 9%),AUC was 0. 919,The CA125 value of 76. 4U/L at follow-up was the best cut-off to predict platinum response with a sensitivity of 95 . 2% and a specificity of 90 . 3% ( PPV=85%and NPV=87. 5%),AUC was 0. 876. The AUC of HE4 at follow-up was also larger than that of CA125 . Logistic multivariate regression analysis revealed that serum HE4 level at follow-up was significantly related to platinum resistant(P=0. 027). Conclusion:Serum HE4 level at follow-up could predict the response of platinum-based first-line chemotherapy in ovarian cancer.关键词
