头颈部256层CTA与彩超对颈动脉颅外段病变急性脑梗死的诊断价值
Diagnostic value of head and neck 256-layer CTA and color Doppler ultrasound in external carotid arterial lesion and acute cerebral infarction

Abstract
Objective To study the diagnostic value of head and neck 256-layer CT angiography(CTA) and color Doppler ultrasound in extracranial internal carotid arterial lesion and acute cerebral infarction. Methods The imaging examination results in 216 patients with acute cerebral infarction complicating extracranial internal carotid arterial lesion in our hospital from May 2012 to October 2015 were retrospectively analyzed. The detection situation ,specificity and sensitivity were compared between the two imaging methods of the head and neck 256-layer CTA and color Doppler ultrasound. Results The detection rate of CTA for the mild and moderate stenosis of extracranial internal carotid artery was significantly superior to that of color ultrasound (P<0.01);while which for severe stenosis and complete occlusion vessel had no statistical difference between CTA and color ultrasound (P>0.05);the consistency of CTA and color ultrasound for diagnosing mild and moderate extracranial internal carotid arterial stenosis was lower(κ=0.02,0.04),while which for diagnosing severe stenosis and complete occlusion was higher(κ=0.35,0.48);the detec-tion rate of these two examination methods for diagnosing blood vessel plaque at bifurcation site of common carotid artery and carotid artery had no statistical difference(P>0.05);the detection rate of CTA for plaques of internal carotid artery and external carotid artery was significantly superior to that of color ultrasound,the difference was statistically significant(P<0.01). Conclu-sion Head and neck 256-layer CTA has more superiority in detecting extracranial internal carotid arterial plaque and stenosis than color ultrasound and has higher clinical application value.关键词
