Abstract
Objective:To evaluate the efficiency and safety of endoscopic and open surgical thyroidectomy in patients with thyroid diseases.Method:Data about controlled clinical tirals of comparing endoscopic thyroidectomy with open surgical thyroidectomy for thyroid diseases was retrieved from electronical databases of Pubmed,Embase,CBM,CNKI and VIP based on the method guidelines for systematic reviews of Cochrane from 1999 to 2015.RevMan 5.3 software was used for data analysis.Result:A total of 11 literatures (1326 cases for thyroid diseases) were selected. The Meta analysis showed that compared to open surgical thyroidectomy,the endoscopic thyroidectomy failed to shorten the operation time (WMD=9.99,95%CI:-3.98-23.95,P=0.16), reduced the intraoperative drainage(WMD=6.10,95%CI:-5.73-17.92,P=0.31),but could significantly reduce the intraoperative blood loss(WMD=-22.96,95%CI:-28.88--17.04,P<0.000 01),shorten the postoperative recovery time(WMD=-0.62,95%CI:-0.98--0.26,P=0.0007),reduced the postoperative complications(OR=0.35,95%CI:0.24-0.53,P<0.000 01).Conclusion:Compared with traditional open surgery,clinical effect of endoscopic thyroidectomy have some advantages,such as less blood loss,shorter time of postoperative recovery,low rate of postoperative complications.However,the trails available for this systematic review are limited,so it should be further studied in the future due to lack of high-quality,multi-center and large-scale RCTs.关键词
