结核分枝杆菌耐链霉素和乙胺丁醇基因的突变特征分析
Analysis on the Characteristics of Mutation Gene of Streptomycin and Ethambutol Drug-resistance in Mycobacterium Tuberculosis

Abstract
Objective:To study the gene mutation of Streptomycin-resistant and Ethambutol-resistant Mycobacterium tuberculosis strains and their effect on the in vitro minimal inhibitory concentration(MIC).Method:140 Mycobacterium tuberculosis strains isolated from our hospital were selected as the objects, which contained 40 Streptomycin-resistant strains,20 Streptomycin-sensitive strains,60 Ethambutol-resistant strains and 20 Ethambutol-sensitive strains. The gene mutation of the strains were detected by gene chip method,the in vitro MIC of all isolates with different gene mutations were detected and compared. Result:The mutation of rpsL or rrs gene was not found in the sensitive strains,the mutation rate of rpsL gene was 62.5%,the rrs gene was 27.5%,and the common mutation rate of the two genes was 10.0%;The MIC of rpsL gene was (70.2±17.1)g/ml,the MIC of rrs gene was (64.2±15.4)g/ml,the difference was not statistically significant (P>0.05).The mutation rate of embB gene in resistant strains was 46.7%,and which in sensitive strains was 40.0%,the difference was not statistically significant on(P>0.05).According to the result,the sensitivity,specialty,PPV,NPV,agreement rate and Kappa value of gene chip method were 46.7%,60.0%,77.8%,27.3%,50.0%,and 0.05 respectively.Conclusion:There is no significant difference in the SM in vitro MIC between rpsL and rrs mutations,the same as the embB306 gene mutations between EMB drug-resistance and sensitive strains.The diagnostic value of gene chip method for the detection of embB gene mutation is not significant.关键词
