护士主导的连续性照护模式在高龄慢性病病人中的应用效果
Application effect of nurse led continuous care model in elderly patients with chronic diseases
摘要
Abstract
Objective:To probe into the effects of nurse led continuous care model in elderly patients with chro-nic diseases.Methods:A total of 100 elderly patients with chronic diseases were randomly divided into interven-tion group(n=50)and control group(n=50).The intervention group received the continuous care model.The control group received usual community health service.Then compared changes of quality of life and treatment compliance,and satisfaction degree of the two groups patients.Results:After 6 months’intervention,changes of quality of life and treatment compliance,and satisfaction degree of the intervention group patients were better than that of the control group(P<0.05).Conclusion:The nurse led continuous care for elderly patients with chronic disease could help elderly patients to improve treatment compliance and quality of life,and to increase their satisfaction degree to community care.关键词
