摘要
Abstract
Objective:To explore the the effect of reduction and internal fixation of mid-and-low mandib-ular condylar fracture through the approach of the surface of the masseter muscle parotid gland leading-edge be-tween the facial nerve.Methods:Thirty-seven patients(43 fractures) with mid or low condylar fractures were di-vided into two groups.Group A:16 patients(19 fractures) were underwent open reduction and internal fixation through a pre-auricular incision passing parotid access.Group B:21 patients(24 fractures) were underwent open reduction and internal fixation through a around mandibular angle skin incision,the surface of the masseter mus-cle parotid gland leading-edge between the facial nerve approach.Compared the clinical effect of two groups.Re-sults:B group,postoperative 2 days occluding relation recovery,salivary fistula occurred and after operation one month,when the mandibular movement joints,local pull discomfort is better than that of group A,the difference was statistically significant( P<0.05);Facial nerve dysfunction、postoperative CT three dimensional reconstruc-tion of bone stump reduction and subjective satisfaction with postoperative 1 month there was no statistically sig-nificant difference(P>0.05).Conclusion:The surface of the masseter muscle parotid gland leading-edge be-tween the facial nerve approach can obtain ideal operative field,and easy to protect the facial nerve and fracture fixation,without separation of parotid gland,salivary fistula and the risk of facial nerve injury is greatly reduced, and can be used to treat such as comminuted fractures of mandibular ramus etc.more complicated fracture.This is one of the security effective operative access.关键词
