甲氨蝶呤联合米非司酮保守治疗内生型剖宫产瘢痕妊娠的有效性及安全性探讨
Analysis on effect and safety of methotrexate combined with mifepristone in conservative treatment of endogenous cae-sarean scar pregnancy

Abstract
Objective To investigate the effect and safety of methotrexate (MTX) combined with mifepristone in the con-servative treatment of endogenous caesarean scar pregnancy(CSP). Methods The clinical data in 37 inpatients with CSP in our hospital from January 2013 to June 2015 were retrospectively analyzed and divided into the treatment group (27 cases) and control group (10 cases)according to different treatment methods. The treatment group was performed the uterine aspiration guided by B-ultrasound after receiving the conservative therapy of MTX combined mifepristone ,while the control group only received the u-terine aspiration guided by B-ultrasound. The treatment success rate,operative time,intraoperative bleeding volume,postoperative hospital stay length,time of recovering to normal bloodβ-HCG and incidence rate of adverse reactions were compared between the two groups. Results The treatment success rate in the treatment group was higher than that in the control group ,the difference was statistically significant[77.78%(21/27)vs. 40.00%(4/10),χ2=4.752,P<0.05];the operative time and postoperative hospitaliza-tion time in the treatment group were(28.13±3.93)min and(5.23±1.45)d,which were lower than(36.52±4.65)min and (7.61± 1.67)d in the control group,the differences were statistically significant(t=5.492,4.258,P<0.05);the intraoperative bleeding vol-ume,recovery time of bloodβ-HCG and incidence rate of adverse reactions had no statistical differences between the two groups (P>0.05). Conclusion The conservative therapy of MTX combined with mifepristone in treating endogenous CSP can increase the treatment success rate,effectively shortens the hospitalization duration,has good safety and is suitable for the promotion in pri-mary hospitals.关键词
