25G细针小剂量布比卡因腰麻在肛肠科手术中的应用效果及安全性研究
Study on Application Effect and Safety of 25G Spinal Needle with Ultra Low-dose Bupivacaine Spinal Anesthesia in Anorectal Surgery

Abstract
Objective:To observe and analyze the clinical effect and safety of 25G spinal needle with ultra low-dose Bupivacaine spinal anesthesia in anorectal surgery.Method:From March 2014 to April 2015, 342 cases of anorectal diseases in our hospital were selected as the research objects,they were randomly divided into the control group and the observation group,each group had 171 cases.The control group was given conventional anesthesia treatment which was 22G spinal needle wish regular doses Bupivacaine,while the observation group was given 25G spinal needle wish ultra low-dose Bupivacaine treatment.The operative difficulty index,anesthesia efficacy,analgesia onset time,recovery situation,sensory blockade plane,degree of motor block,duration of anesthesia,adverse reaction and degree of satisfaction in two groups were compared. Result:The success rate of primary anesthesia in the control group and observation group were respectively 73.1%(125/171) and 80.7%(138/171),the differences were statistically significant(P<0.05).The total effective rate of anesthesia in the control group and observation group were respectively 96.5%(165/171)and 100%(171/171).After treatment,recovery time of lower limb muscle strength,autonomic micturition recovery time,duration of anesthesia in the control group were respectively (2.35±0.63)h,(16.32±4.52)h and (87.56±17.69)min,the three indexes of observation group were respectively,(1.56±0.22)h,(11.78±2.02)h and (60.34±13.21)min,the differences were statistically significant(P<0.05).The degree of motor block, adverse reactions and patient satisfaction rate in the observation group were better than those of the control group, the differences were statistically significant(P<0.05).Analgesia onset time and maximum sensory block level <br> (median)of the two groups had no significant differences(P>0.05).Conclusion:25G spinal needle with ultra low-dose Bupivacaine spinal anesthesia has significant efficacy in the anorectal diseases surgery,has advantage of fewer complicalions,faster recovery and higher patient satisfaction rate,it is worthy of clinical promotion.关键词
