2012年至2015年临床常见病原菌分布及耐药趋势分析
Analysis of pathogen distribution and drug resistance at a secondary hospi-tal in Chongqing 2012 -2015

Abstract
Objective:To investigate the pathogen distribution and drug resistance at a secondary hospital in Chongqing (2012-2015),so as to provide scientific basis for reasonable clinical use of antibiotics.Methods:The records of clinical pathogenic bacteria which were isolated from clinical specimens during 2012 -2015 were dynamically compared and analyzed on the pathogen distribution and drug resistance of the ward.Pathogenic bacteria identification and drug susceptibility tests were performed with a Zhuhai Biotech DL-II compact automatic identification system and data were analyzed using WHONET 5.6 software.Results:A total of 4 619 strains of pathogens were separated during the four years,including 1338 strains of Gram-positive pathogens,2 974 strains of Gram-negative pathogens and 307 fungus.The Pseudomonas aeruginosa,Escherichia coli,Klebsiella pheumoneae,and Acinetobacter baumannii ranked the top four Gram-negative pathogens,and the common Gram-positive pathogens were staphylococcus,streptococcus and enterococcus. The same pathogen derived from different specimens had different trend of drug susceptibility tests.In summary,the trend of ESBLs (escherichia coli,Klebsiella pneumoniae)positive rate rose first and fell later.Piperacillin and ampicillin displayed unsatisfied antibac-terial activity against Enterobacteriaceae.Efuroxime,Cefotaxime,Cefoperazone and Ceftriaxone displayed ordinary antibacterial activity against Enterobacteriaceae.Cefoperazone,Sulbactam displayed desirable antibacterial activity against Acinetobacter baumannii;ceftazi-dime had good activity against Escherichia coli,Pseudomonas aeruginosa,Klebsiella pneumoniae,but little activity against Acinetobacter baumannii.Staphylococcus displayed desirable sensitivity against vancomycin,linezolid,nitrofurantoin,teicoplanin,but high rate of drug-resistance against penicillin,sulfamethoxazole,aminoglycosides.The multi-drug resistance of enterococcus was high.Conclusion:There are many kinds of multi-drug resistant pathogenic bacteria in our hospital.Drug monitoring data is important for clinical treatment and rational use of antibiotics.关键词
