内镜下切除与传统开腹根治术治疗早期胃癌的预后情况及创伤程度评估
Assessment of prognosis and trauma extent in endoscopic resection and traditional open rad-ical resection treatment of early gastric cancer

Abstract
Objective:To study prognosis and trauma extent in endoscopic resection and traditional open radical resec-tion treatment of early gastric cancer.Methods:Patients with early gastric cancer who received surgical resection in our hospital from January 2014 to December 201 5 were selected and randomly divided into endoscopy group and laparotomy group who re-ceived endoscopic resection and traditional open radical resection respectively.Levels of perioperative serum tumor markers, trauma-related neuroendocrine indicators as well as co-stimulatory molecules CD25,CD28 and ICOS were detected.Results:One month,three months and one year after operation,serum gastrin-1 7 (G-1 7),thymidine kinase (TK)-1,carcino-embryon-ic antigen (CEA),carbohydrate antigen (CA)1 99,CA125 and CA724 levels of both groups were significantly lower than those before operation (P <0.05)and serum G-1 7,TK-1,CEA,CA1 99,CA125 and CA724 levels between two groups were with-out significant difference (P >0.05);serum adrenocorticotrophic hormone (ACTH),cortisol (Cor)and β-endorphin (β-EP) levels of endoscopy group 1 d after operation were not significantly different from those before operation (P >0.05),and serum ACTH,Cor andβ-EP levels of laparotomy group 1 d after operation were significantly higher than those before operation (P <0.05);1 week after operation,CD25 + ,CD28 + and ICOS proportion in peripheral blood of endoscopy group were not signifi-cantly different from those before operation (P >0.05),and CD25 + ,CD28 + and ICOS proportion in peripheral blood of lapa-rotomy group were significantly lower than those before operation (P <0.05);1 month after operation,CD25 + ,CD28 + and ICOS proportion in peripheral blood of both groups were significantly higher than those before operation (P <0.05)and post-operative CD25 + ,CD28 + and ICOS proportion in peripheral blood were not significantly different between two groups (P >0.05).Conclusions:Endoscopic resection treatment of early gastric cancer has equivalent prognosis to traditional open radical resection and causes less trauma.关键词
