伤椎椎体内植骨结合后外侧植骨内固定治疗胸腰椎爆裂骨折的疗效

摘要
Abstract
Objective To study the efficacy of transpedicular bone grafting combined with posterolateral fusion in the treatment of thoracolumbar burst fractures. Methods Group A(20 patients) with thoracolumbar burst fractures were treated with transpedicular bone grafting and posterolateral fusion, combined with spinal internal fixation fixation. Group B(22 paitents) with posterolateral fusion were studied comparatively. Postoperative radiographic index,visual analogue scale( VAS) and spinal nerve function were compared between the two groups. Results All the cases were followed up for 12~18(15 ± 2. 6)months. There were significant differences on the reduction of the anterior height of injuried vertebral body and the Cobb angle between the two groups at pre- and post-operaion and the last follow-up (P<0. 05). Compared between the two groups,data did not show significant difference postoperatively(P>0. 05). However, at last follow-up, there was significant difference between the two groups(P<0. 05). VAS was significant-ly improved at post-operation and the last follow up(P<0. 05),while there wasn′t significant difference between the two groups (P>0. 05). Spinal nerve function of the two groups were all significantly improved at last follow-up(P<0. 05),while there wasn′t significant difference between the two groups (P>0. 05). Conclusions Vertebral pedicle screw fixation with transpedicular bone graft and posterolateral fusion can not only reduce verbebral fracture satisfacto-rily,increase the stability of spine,but also prevent long-term vertebral height and cobb angle loss of correction,de-crease the possibility of the breakage of the internal fixation postoperatively.关键词
