肝硬化门静脉高压症患者肝脏血流动力学指标与门静脉压力的相关性研究
Correlation Between Liver Hemodynamic Indicators and Portal Vein Pressure in Patients With Cirrhotic Portal Hypertension

Abstract
Objective To investigate the correlation between liver hemodynamic indicators and portal vein pressure (PVP)and explore the feasibility of Doppler ultrasound in the noninvasive assessment of PVP in patients with cirrhotic portal hypertension(PHT). Methods From December 2012 to December 2014,we enrolled 30 patients who received TIPS treatment for cirrhotic PHT in the Second Affiliated Hospital of Nanchang University as PHT group,and enrolled 30 healthy people who received physical examination in the same period in the hospital as control group. Doppler ultrasonic tester was adopted to determine the hemodynamic indicators of liver including PVD,PVV,PVF,SVD and SVV. Child-Pugh grading was used to evaluate the liver function of patients in PHT group;during TIPS,PVP of patients in PHT group was measured by glass tube manometer. Results PHT group was higher in PVD,PVF and SVD and lower in PVV and SVV than control group( P <0. 05). Cirrhotic PHT patients with different liver function grades were significantly different in PVD,PVV,SVD and SVV(P< 0. 05). PVP of cirrhotic PHT patients of grade A,grade B and grade C was(34. 6 ± 2. 5),(41. 2 ± 3. 6),(42. 0 ± 3. 6) cm H2 O respectively,and the differences were significant( F = 17. 598,P < 0. 001);patients of grade B and grade C were higher than patients of grade A in PVP(P < 0. 001). For patients of grade A in liver function,PVD was positively correlated with PVP(r = 0. 584,P < 0. 05),and PVV,PVF,SVD and SVV had no linear correlation with PVP(r = - 0. 412,0. 270, 0. 243, - 0. 328;P > 0. 05 ). For patients of grade B in liver function,PVV was negatively correlated with PVP ( r= - 0. 631,P < 0. 05),and PVD,PVF,SVD and SVV had no linear correlation with PVP(r = - 0. 171,0. 370,0. 183,- 0. 322;P > 0. 05). For patients of grade C in liver function,PVF was positively correlated with PVP( r = 0. 878,P <0. 05),and PVD,PVV,SVD and SVV had no linear correlation with PVP(r = 0. 503,0. 472,0. 320,0. 264;P > 0. 05). Conclusion Liver hemodynamic indicators,including PVD,PVV and PVF,have certain correlation with PVP of cirrhotic PHT patients and may be potential indexes for the evaluation of PVP.关键词
