前列腺癌经直肠超声图像特征与其危险度的关系研究
Relationship Between Image Features of Transrectal Ultrasound of Prostate Cancer and Its Risk

Abstract
Objective To explore the relationship betweenimage features of transrectal ultrasound(TRUS)of prostate cancer( PCa)and its risk. Methods 102 PCa patientswho had received treatment in the Tenth People's Hospital of Tongji University from May 2013 to March 2015 and met the inclusion criteria,were retrospectively selected as the subjects of study. Pathological diagnosis was given to patients after needle biopsy. By TRUS and MRI examination,patients were divided into low - risk PCa,moderate - risk PCa and high - risk PCa according to prostate specific antigen(PSA),Gleason score( ﹤ 7)of biopsy pathology or clinical stages. The relationship between TRUS image features( including bilateral symmetry of prostate, integrity of the capsule,demarcation definition of the internal and external glands,whether there wereprostate nodules,and the size,border,echo and blood flow of the nodulesof patients with prostate nodules)and PCa risks was observed. Results Among the 102 PCa patients,19 were low - risk PCa(18. 6% ),29 moderate - risk(28. 4% )and 54 high - risk(53. 0% ). The PCa risk of patients with asymmetric bilateral sides of prostate,incompleteand unclear boundary between internal and external glands is significantlyhigher than that of patients with symmetric sides of prostate,complete capsule and clear boundary between internal and external glands(P ﹤ 0. 05). There was no significant difference in the risk of PCa between patients with or without nodules (P ﹥ 0. 05). There were 6 low - risk PCa patients,18 moderate - risk,and 32 high - risk among the 56 PCa patients with prostate nodules. The PCa risk of patients with nodules≥10 mm was higher than that of patients with nodules ﹤ 10 mm,and the risk of PCa of patients with nodular blood flow of Ⅱ - Ⅲgrades was higher than that of patients with nodular blood flow of 0 - I grade(P ﹤ 0. 05);there was no significant difference in the PCa risk of patients with different definitions of nodule boundaries and nodular echoes(P ﹥ 0. 05). Conclusion The risk of PCa can be evaluated by the symmetry of bilateral sides of prostate, the integrity of the capsule,the demarcation definition of internal and external glands,the size of the nodules and the blood flow of the nodules in TRUS image features. Therefore,it is suggested that TRUS examination resultscan be taken as the reference for clinical treatment.关键词
