住院药房常见用药差错原因与防范措施分析
Analysis of the causes and preventive measures of common medication errors in inpatient pharmacy

Abstract
Objective:To analyze the causes and preventive measures of common medication errors in inpatient pharmacy. Methods:90 cases of medication errors events were as the study objects.The different error types were classified.The error causes were summarized.The corresponding preventive measures were formulated according to this.Results:90 cases of medication error events included the drug hair wrong(44.44% ),number hair wrong(24.44% ),wards confusion(15.56% ),omission errors(13.33% ), quality error(2.22%).The causes include drug, pharmacists, processes,distribution,environment this five factors.Conclusion:There are many reasons that lead to medication errors in inpatient pharmacy.Clearing these factors and adopting corresponding preventive measures can improve the safety and accuracy of medication.关键词
