64层螺旋CT血管成像颅内小动脉瘤的诊断价值研究

Abstract
Objective:To evaluate the diagnostic value of 64 slice spiral computed tomography angiog-raphy ( CTA) on detecting intracranial small aneurysm. Methods:CTA examination results from 264 patients who were suspected of having intracranial aneurysm and vascular disease were retrospectively analyzed and vol-ume-rendering rotation digital subtraction angiography, surgical findings or combination of them served as golden standard of examination to evaluate the diagnostic value of 64 slice spiral computed tomography angiog-raphy on detecting small intracranial aneurysm. Results:84 small intracranial aneurysms from 71 patients un-der the golden standard assessment were found. According to the evaluation of patients, diagnostic accuracy, sensitivity and specificity of 64 slice CTA on small intracranial aneurysm were 98. 2%, 98. 6%, 97. 6%, re-spectively. PPV was 98.6% and NPV was 97.6%. According to the evaluation of aneurysm, diagnostic accura-cy, sensitivity and specificity of 64 slice CTA on small intracranial aneurysm were 96.8%, 97.6%, 95.1%, respectively. Positive predictive value was 97.6% and negative predictive value was 95.1%. Both of two false positive cases and two false negative cases existed among the microaneurysm patients ( diameter less than 3mm) . Conclusion:64 slice CTA was a reliable kind of imageological screening examination method on de-tecting small intracranial aneurysm. Volume-rendering rotation DSA examination remains necessary to be used for these patients who had SAH histories and negative results of CTA detection.关键词
