Abstract
Objective To explore the surgical treatment options for patients with temporal lobe or medial temporal lobe cavernous malformation with epilepsy as the initial symptom.Methods A retrospective analysis of 49 cases of temporal lobe or medial temporal lobe cavernous malformation with epilepsy as the initial symptom admitted into our hospital from 2009 to 2013 and treated by surgery were carried out, of which generalized tonic-clonic seizures in 26 cases, complex partial seizures in 23 cases.The 49 cases were divided into two groups, Group A with 20 patients taken simple excision of lesion, Group B with 29 patients taken extensive resection of lesions and temporal lobe or inner structure of the epileptic focus under the guidence of the intraoperative EEG monitoring results.Results The Engel effect rating was adapted.In group A, GradeⅠ was 70%, GradeⅡ20%, GradeⅢ 10%.All the pathological findings were cavernous malformation.In Group B, GradeⅠwas 89.66%, GradeⅡ6.9%, GradeⅢ3.44%.All pathological results were cavernous malformation, including 25 cases with hippocampal sclerosis, 3 cases with cortical dysplasia.There was no severe intraoperative and postoperative complications and death.Conclusions Seizures can be effectively controlled by actively surgical treatment in patients with temporal lobe or mesial temporal lobe cavernous malformation with epilepsy as the initial symptom.Seizure may be more effectively controlled by selective extensive resection of the epileptic focus under the guidence of preoperative and intraoperative EEG results.Temporal lobe and medial temporal lobe cavernous malformations patients may be with dual pathology.关键词
