Abstract
Objective To explore the effect of improved midwifery vaginal delivery on reducing episiotomy rate.Methods According to random number table method, 3 400 puerperas admitted in Chengdu Center of Women and Children Hospital during January to December in 2014 were divided into observation group and control group, and 1 700 cases were recruited in each group.The control group received traditional way of midwifery, while the observation group received improved midwifery vaginal delivery.The maternal episiotomy rate and laceration, postpartum bleeding volume, pain degree, neonatal Apgar score and hospitalization time were compared between two groups. Results The perineum of cases in the observation group was significantly better than that in the control group, and the perineum complete rate and episiotomy rate in the observation group were significantly lower than those in the control group (χ2 value was 23.100 and 11.442, respectively, both P<0.01), but the incidence of degree I laceration of perineum was significantly higher than that in the control group (χ2 =17.553, P<0.01).There was no significant difference in the incidence of degree II laceration of perineum (χ2 =0.901, P>0.05).The postpartum hemorrhage amount, VAS score and the length of hospitalization in the observation group were superior to those in the control group ( t value was 52.128, 23.272 and 39.637, respectively, all P<0.05 ) , but there was no significant difference in newborn Apgar score between two groups (t=0.000, P>0.05).Conclusion The improved method can better protect perineum and reduce episiotomy rate, which has high clinical value and is worthy of clinical application.关键词
