Effect of Discharge Planning for Elderly Patients with Chronic Heart Failure

Abstract
Objective To establish a discharge planning for elderly patients with chronic heart failure, to evaluate its effect and to provide a reference mode for the management of chronic disease in China. Methods The patients meeting the inclusion criteria were randomly divided into control group and experimental group. Patients in control group received routine care and discharge guidance while a discharge planning program was performed in experimental group. Hospital stay, medical costs, 6-min walk distance 1, 3, 6 and 12 month after discharge and patients ’ quality of life were observed in two groups. Results There were less medical cost and hospital stay in experimental group. 6-min walk distance and quality of life scoring 1 month after discharge indicated no significant difference (P>0.05) but they did 3, 6, and 12 month after discharge (P<0.05). Conclusion Discharge planning helps to shorten hospital stay and reduce medical expenses for elderly patients with chronic heart failure and it benefits the improvement of exercise tolerance and quality of life.关键词
