健康服务整合：以德国北部痴呆症健康管理为例
Integrated healthcare:an example in dementia care management from northern Germany
摘要
Abstract
This paper takes an integrated model of dementia care management from Western Pomerania as an example. Dementia healthcare resources in this area,intervention process,intervention standard and computerized intervention management system of the model were represented and analyzed. Based on these,it was concluded that integrated healthcare was effective for patients with dementia living at home,and their caregivers needed to be supported and educated. In addition,computerized system could promote integrated healthcare. Finally,some suggestions were made according to China’s practice. It is suggested that the government improve healthcare infrastructure for the elderly in primary care settings,and strengthen collaborative care between professional nurses and home caregivers. It is also suggested that the caregivers be educated on caring for patients with dementia and supported them in physical and mental health,working conditions and social lives. Morever,it is proposed to explore informatization management of dementia care.关键词
