Abstract
It mainly summarized the definition,risk factors and harm of H type hypertension,reviewed research progress on hospital and community continuity of care on H type hypertension,in order to improve the attention to H type hypertension,early detect and implement the comprehensive,systematic and effective continuity of care and to reduce the risk of cardiovascular and cerebrovascular events.关键词
