自护理论结合医护协同模式在精神疾病病人社区康复中的应用
Self care theory with collaborative care model applied in community based rehabillitation of patients with mental illness

Abstract
Objective:To probe into the influence of Orem self care theory combined with collaborative care in-tervention model on community based rehabilitation of mental illness patients in stable stage when they re-turned family.Methods:A total of 136 mental illness patients in stable stage from June 2014 to June 2015 in a community service center were randomly divided into experimental group and control group 68 cases in each. The patients in control group was treated with telephone interviews and home health care visits,and the patients in experimental group were treated with Orem self care theory combined with collaborative care model for management for a period of 1 2 months.The patients of two groups were evaluated at the time before interven-tion,6 months and 1 2 months after intervention by using the Follow up service for patients with severe mental illness,Activities of Daily Living Score(ADL),and WHO Quality of Life BREF(WHOQOL BREF),and re-currence rate and rehospitalization rates were collected after 6 months and 1 2 months.Results:After 6 months and 1 2 months of intervention,the patients’daily life activities compliance and medication compliance in experi-mental group were better than those in control group(P<0.05 or P<0.01);the patients in experimental group had higher QOL scores(P<0.05 or P<0.01);the instrumental activities of daily living score in experimental group was lower than that in control group(P<0.01);after 12 months of intervention,the physical activity score in experimental group was lower than that in control group(P<0.01),and the recurrence rate and the re hospitalization rate were all lower than those in control group(P>0.05).Conclusion:Orem self care theory combined with collaborative care intervention model could significantly improve mental illness patients’com-munity rehabilitation effects compared with conventional management model,and it could help mental illness patients to have a better recovery of daily activities and family function.关键词
