基层医院开展全胸腔镜肺叶切除术治疗早期肺癌的临床研究
Application study of total thoracoscopic lobectomy for treatment of early lung cancer in primary hospital

Abstract
Objective To compare the clinical indicators of patients undergoing complete total thoracoscopy lobecto -my and conventional lobectomy , and to explore the feasibility and application value of thoracoscopic operation .Methods To-tally 68 patients with lung cancer in this study from December 2011 to June 2015 were enrolled , 35 cases undergoing complete total thoracoscopy lobectomy ( study group ) , and 33 cases undergoing complete conventional lobectomy ( control group ) . Differences of numbers of lymph nodes removal , operation duration , volumes of intraoperative bleeding , length of postoperative hospital stay, surgery cost, and incidence rates of postoperative complication were compared .Patients of study group's total pleural drainage was lower than that in control group , but the difference was not statistically significant ( P >0.05);compari-son of the 2group 's lymph node dissection station and number of lymph node dissection showed that the difference was not sta -tistically significant ( P >0.05); two groups of operation cost is close to the same , no significant differences were found (P >0.05).Results All procedures were carried out smoothly without serious complications .Postoperative extubating time, the length of the incision , blood loss, the average length of stay of study group were significantly lower than those of the control group ( P <0.05), while there was no significant differences between the two groups in lymph nodes dissection number , postoperative drainage and surgery cost ( P >0.05).However, the operation time of the control group were significantly shor-ter than that of the study group ( P <0.05).Conclusion Compared with traditional thoracotomy lobectomy , total thoraco-scopic surgery can shorten hospital stays , reduce surgical trauma ,which is worthy of clinical application .关键词
