A meta-analysis on curative efifcacy of invasive and noninvasive sequence mechanical ventilation for acute respiratory distress syndrome

Abstract
Objective To evaluate the curative efficacy of invasive and noninvasive sequence mechanical ventilation for acute respiratory distress syndrome (ARDS).Methods A review of randomized controlled trials with meta-analysis performed by searching databases of PubMed, Ovid, EBSCO, China Biology Medicinedisc (CBMdisc), VIP database (VIP), Wan Fang data. Randomized controlled trails compared invasive and noninvasive sequence mechanical ventilation with conventional invasive ventilation in patients with ARDS were eligible for inclusion.Results Eight trials were included, involving 404 ARDS patients. Meta-analysis showed that invasive and noninvasive sequence mechanical ventilation compared with control group, was found shorter total duration of ventilation support [(mean difference,MD)=5.85, 95%confidence interval (CI) (3.62,8.09),P<0.001] and hospital stays in intensive care unit (ICU) were serially determined [MD=5.95, 95%CI (5.16,6.73),P<0.001] and lower ventilator associated pneumonia (VAP) incidence [relative risk (RR)=0.25, 95%CI(0.16,0.39),P<0.001] and mortality rate [RR=0.35, 95%CI(0.22,0.56),P<0.001].Conclusions Invasive and noninvasive sequence mechanical ventilation may improve the prognosis of ARDS patients to some extent.关键词
