BiPAP联合普米克令舒／可必特治疗老年AECOPDⅡ型呼吸衰竭的临床研究

Abstract
Objective:To explore the treatment effect of elderly AECOPD with type Ⅱ respiratory failure by BiPAP combined with Pulmicort /Combivent treatment,observe their prognosis.Methods:98 AECOPD patients were randomly selected in our hospital from March 2014 to October 2015,they were randomly divided into the study group(49 cases)and the control group(49 cases).And the control group were treated with BiPAP therapy on the basis of conventional therapy,the study group were given Pulmicort /Combivent on the basis of the control group.The oxygen saturation (SaO2 ),blood oxygen pressure (PaO2 ),carbon dioxide partial pressure (PaCO2 ),PH value were detected before and after treatment 1 d and 3d.Recorded the intubation rate,the mortality and the respiratory support time,improve blood time and stay hospital time.Results:SaO2 ,PaO2 ,PH value of the study group and the control group after treatment 1 d and 3d were higher than before treatment,PaCO2 lower than before treatment,the difference was statistically significant(P <0.05).SaO2 ,PaO2 of the study group after treatment 3d respectively were (89.45 ±5.12)% and (78.69 ±5.04)mmHg,higher than the control group (87.33 ±4.32)% and (75.12 ±4.64)mmHg,PaCO2 of the study group after 3d treatment was (48.52 ±4.31 ) mmHg,lower than the control group (5 1 .30 ±4.27)mmHg,the difference was statistically significant (P <0.05).The intubation rate of the study group was 4.08% (2 /49),lower than the control group 14.29% (7 /49),the difference was statistically significant (P <0.05).The respiratory support time,improve blood time,hospital time of the study group were (3.98 ±0.86)d,(4.70 ±0.81 )d and (17.31 ±3.34)d,lower than the control group,the difference was statistically significant (P <0.05 ).Conclusion:Application of Bi-PAP United Pulmicort /Combivent treatment of the elderly AECOPD patients with Ⅱ type respiratory failure can reduce re-intubation rate,improve arterial oxygen condition,shorten treatment and improvement time.关键词
