肠内营养途径对危重病人胃液pH值及呼吸机相关性肺炎发生的影响研究
Study on influence of enteral nutrition routes on gastric pH and ventilator associated pneumonia in critically ill pa-tients

Abstract
Objective:To probe into the influence of two different enteral nutrition support routes on the pH val-ue of gastric juice in patients with invasive mechanical ventilation and its correlation with the occurrence of vent-ilator associated pneumonia(VAP).Methods:A total of 40 ICU patients admitted to Tianjin First Central Hospital from September 2014 to November 2015 were randomly divided into two groups:control group and ob-servation group.All the patients were given the same Nutritional preparations and antacids.The patients in con-trol group were given intragastric feeding through the nasogastric tube,while the patients in observation group were given gastrointestinal drainage through the gastrointestinal double lumen tube.The change of gastric pH in fasting state and the occurrence of VAP were compared undergoing enteral nutritional support between the two groups.Results:The average gastric pH in control group was between 4 and 7 while the average gastric pH in observation group was between 4 and 8 in fasting state,the difference was statistically significant(P <0.01);the incidence of VAP of observation group was lower than that in control group,but the difference was not sta-tistically significant(P =0.127).There was statistically significant difference between the two groups in the in-cidence of VAP and the incidence of late onset VAP(P <0.05).Conclusion:The use of gastrointestinal double lumen combined with gastric drainage as enteral nutrition support route was better than the nasogastric tube, and it had certain clinical significance in controlling early VAP.关键词
