Research progress on status quo of continuity of care of chronic diseases under hospital community linkage
摘要
Abstract
It introduced the concept,urgency of hospital community linkage and its status quo at home and a-broad.It pointed out the lack of hospital community linkage in China,and disconnection and fault phenomenon of nursing service in patients with chronic diseases after discharge.It put for ward that future research should a-chieve continuous informazation,formulate hospital community linkage mechanism under Chinese national condition,realize continuity of information,relationship and management of patients with chronic diseases be-tween hospital and community to enhance quality of continuity of care of patients.关键词
