摘要
Abstract
To provide a pharmaceutical care model for the practice of "patient-centered clinical" pharmacy so as to ensure safe,effective and economical clinical medication.In this paper,one case of severe pneumonia patients who had been treated with anti-infection was monitored by clinical pharmacist.Clinical pharmacists had timely follow-up to understand the patient’s condition changes,and actively analyzed the results of the auxiliary examination,and provided recommendations for the treatment of patients.It was noticeable that clinical pharmacists could cooperate with physicians to optimize treatment plan,carry out pharmaceutical care and promote clinical rational drug use.At the same time,clinical pharmacists could improve the pharmaceutical care ability and pharmaceutical service level by analyzing the key cases.关键词
