Abstract
Objective:To compare the advantages and disadvantages of laparoscopic extralevator abdominoperineal excision ( LELAPE) for the resection of low rectal cancer by different approach sequences.Methods:From Feb.2013 to Sep.2015,30 patients with distal rectal cancer undergoing LELAPE at the department of Gastroenterological Surgery,Beijing Hospital were analyzed retrospec-tively.They were divided into two groups.There were 7 males and 7 females with an average age of (61 ±6.4) years old in abdomen priority group.There were 10 males and 6 females with an average age of (63 ±8.2) years old in perineum priority group.The data of two groups were analyzed by t test orχ2 test.Results:The abdomen and perineal operation time was (74.6 ±4.765) min and (55.79 ±5.07) min respectively in abdominal priority group,intraoperative bleeding was (85.0 ±9.4) ml,the postoperative hospital stay was (9.0 ±1.664) d.The abdomen and perineal operation time was (56.88 ±4.44) min and (72.0 ±4.775) min respectively in perine-um priority group,intraoperative bleeding was (80.5 ±11.2) ml,the postoperative hospital stay was (10.0 ±2.191) d.The abdominal operative duration in abdomen priority group was longer than that in perineum priority group (P<0.01),whereas the perineum opera-tive duration in perineum priority group was longer than that in abdomen priority group (P<0.01).There was no significant difference in blood loss volume,complication rate of perineal incision,postoperative hospital stay between two groups (P>0.05).Conclusions:LELAPE by two different approach sequences are all safe and feasible,perineum priority approach seems more appropriate for compli-cated LELAPE procedure,and can further reflect the advantages of this technique.关键词
