Diagnostic and differential values of key signal molecule Wnt3a expressed in the Wnt pathway for hepatocellular carcinoma

Abstract
Objective: To explore the diagnostic values of the key signal molecule Wnt3a expression for hepatocellular carcinoma (HCC). Methods: Circulating Wnt3a levels were determined in a total of 186 patients (80 HCC, 53 liver cirrhosis and 53 chronic hepatitis) and 40 controls by ELISA, and were compared with levels of AFP to evaluate their diagnostic and differential diagnostic values for hepatocellular carcinoma. The expression of Wnt3a in 80 HCC and their surrounding tissues was analyzed by IHC for clinicopathological features. ResuIts:The average levels of serum Wnt3a expression were significantly higher (P<0.001) in the HCC group than those in any other groups of benign liver diseases. Wnt3a expression in HCC patients were closely related (P<0.05) to liver cirrhosis, HBV infection, poor differentiation, and TNM staging. The sensitivity, specificity, accuracy, positive predictive value and negative predictive values of Wnt3a were 92.5, 94.3, 93.2, 96.1 and 89.3%respectively when Wnt3a of 800 ng/L was used as the cutoff value for diagnosis of HCC. When Wnt3a and AFP were combined, the sensitivity rose to 96.3%, and the area under curve in Wnt3a (0.994) was higher than in AFP (0.710) with the ROC curve. The positive Wnt3a with brown staining particles were mainly distributed in cytosol and membrane ofhepatocytes. Higher Wnt3 expression was seen in 71.3%cancerous and 13.8%surrounding tissues, and associated with poor differentiation, cirrhosis, HBV, and high TNM stage (P < 0.01). ConcIusion: As a key molecule in the Wnt signaling pathway, Wnt3a may be used as a specific marker for HCC diagnosis and differential diagnosis.关键词
