Abstract
Objective:To study the effectiveness of clomiphene citrate( CC) for preven-tion of premature LH surge in non-down-regulation ovarian hyperstimulation protocol. Methods:A retrospective cohort study was conducted of 98 cases of normal ovarian reserve patients from April 2013 to Oct. 2014 in our IVF center. All patients were stimulated with CC in combination with gonadotropins ( Gn ) . The main outcome measures included basal luteinizing hormone ( LH) value,LH value on 2 days before HCG administration,LH value on the day before HCG administration,LH value on the day of HCG administration,the average number of oocytes,MII oocytes rate,fertilization rate,available embryo rate,good quality embryo rate,basal endometrial thickness,endometrial thickness on the day of HCG administration, implantation rate, clinical pregnancy rate. Results:Basal LH was(3. 19±2. 17)mIu/ml;LH value on the days before HCG administration was (4. 25±2. 70)mIU/ml;LH value on the day before HCG administration was(4.70±2.53)mIU/ml.LH value on the day of HCG administration was (5.85±3.57)mIU/ml. Basal luteinizing hormone( LH) value. LH value on 2 days before HCG administration,LH value on the day before LH value on the day before HCG administration,LH value on the day of HCG administration were all less than 10mIU/ml. Although LH value showed an increasing trend,there was no premature ovulation. There were 9 cases with high LH level,but this did not reduce oocyte quality,embryo quality and pregnancy outcome. The mean amount of Gn needed was (2142. 23±712. 57) IU. The mean days of stimulation were (10. 4 ±1. 81). The average number of oocytes was (12. 33±6. 29). Mean endometrial thickness on the day of HCG admin-istration was (9. 45±2. 20)mm. Oocyte maturation rate was 86. 8%(1071/1235). Fertilization rate was 74 . 1%( 794/1071 ) . Good quality embryos rate was 52 . 0% ( 412/794 ) . Available embryo rate was 80. 3%(638/794). Oocyte retrieval cycles were 98. The number of fresh em-bryo transfer cycles were 60 cycles. Clinical pregnancy rate was 51. 7%(31/60). Conclusion:CC combined with non-down-regulation ovarian hyperstimulation protocol can effectively prevent premature LH surge in normal ovarian reserve patients. Fresh embryo transfer can be used rou-tinely,and clinical pregnancy rate can be guaranteed.关键词
