Abstract
Objective To observe the curative effect of insulin detemir therapy in patients with type 2 diabetes and the influence on the function of islet beta cell.Methods Seventy-four cases of patients with type 2 diabetes were selected, who were treated in hospital from January 2015 to January 2016, and they were divided into the study group (37 cases) and control group (37 cases).The patients of two groups were all treated with accepting strict diabetic diet, regular exercise, and long acting insulin subcutaneous for intensive therapy.The patients of control group were treated with insulin aspart (0.2 U・kg-1・ d-1 immediately subcutaneous before a meal).While the patients of study group were treated with detemir (based on fasting blood glucose control targets, real time detection to adjust the dose and keep FPG ≤ 6 mmol/L, 2hPG ≤8 mmol/L). The patients of two groups were all treated with continued three months.Results After treatment, 2 groups of FPG, 2hPG, HbA and 1C were significantly lower than those before treatment ( t =8.937, t =14.003, t =8.187 in study group, t =8.664, t =14.064, t =8.091 in control group, P =0.000); there was no significant difference between the 2 groups ( t =0.617, t =0.061, t =0.218, P =0.269, P =0.476, P =0.423) after treatment; 2 the fasting insulin (FIns), 2 h post-prandial insulin (2hIns), fasting C peptide (CPO), postprandial 2 h peptide (CP2) and pancreatic beta cell function index (HOMA-β) were significantly higher than those before treatment (study group: t =7.036, t =9.582, t =7.557, t =7.600, t =7.448,control group: t =18.542, t =13.229, t =11.308, t =20.652, t =12.389, P =0.000); compared with the control group after treatment, FIns, 2hIns, CPO group, CP2 and HOMA P were significantly decreased ( t =12.335, t =7.584, t =5.358, t =8.536, t =6.915, P =0.000); the 2 groups after treatment in patients with multiple food easy to hungry, thirsty drink, Fatigue, limb numbness and pain symptom score were significantly decreased (study group: t =7.036, t =9.582, t =7.557, t =7.449, t =13.229,control group: t =18.542, t =11.308, t =12.389, P =0.000); compared with the control group, the study group scores were significantly reduced (χ2 =12.335,χ2 =7.584,χ2 =5.359,χ2 =6.915, P =0.000); adverse reactions of hypoglycemia and gastrointestinal reaction occurred during the treatment of the 2 groups, but the difference was not statistically significant (χ2 =0.342,χ2 =1.074, P =0.559, P =0.300).Conclusion Insulin det-emir can effectively reduce blood glucose levels in patients with type 2 diabetes, promote insulin secretion, improve beta cell function, safe and reliable.关键词
