硬脑膜网状成形术预防去骨瓣减压术并发症的临床价值
Clinical Value of Dura Reticulate Plasty in Prevention of Complications Induced by Decompressive Craniectomy

Abstract
Objective To evaluate clinical values of dura reticulate plasty in prevention of complications induced by decompressive craniectomy. Methods Surgical methods of dura reticulate plasty in prevention of 29 patients with complica-tions induced by decompressive craniectomy were analyzed, incidence rates of related decompression complications during and after surgery were observed using head CT, and postoperative intracranial pressure ( ICP) detection was performed, and then modified Rankin scale ( mRS) was performed in postoperative 3 months to evaluate recovery conditions of patients. Results No complications such as bone window entrapment syndrome, brain prominentia, bone window hernia, reverse brain displace-ment or intracalvarium subdural effusion were found before and after surgery by head CT, and the above complications did not be seen by unaided eyes during surgery. During postoperative 5-10 d ICP detection for the 29 patients, ICP was less than 15 mmHg in 15 patients, ICP was 15-20 mmHg in 11 patients, and ICP was 20-25 mmHg in 3 patients. Postoperative 3 months mRS scores showed that 4 patients with 0 point, 7 patients with 1 point, 5 patients with 2 points, 5 patients with 3 points, 4 patients with 4 points, 2 patients with 5 points and 2 patients with 6 points, and well recovery rate was 55. 2% (16/29). Conclusion Decompressive craniectomy combined with dura reticulate plasty can effectively decompress, prevent related complications, reduce damage of related nerve function and improve clinical prognosis, but its effects needs to be further con-firmed.关键词
