甲状腺癌根治手术中比较使用LigaSure血管闭合系统和超声刀的单盲前瞻性随机对照临床研究

Abstract
Objective The aim of the study was to compare operative factors, postoperative outcomes, and surgical complications of thyroidectomy using Ligasure vessel sealing system versus the harmonic focus scalpel.Methods The study was a single-blind, randomized controlled trial. It was conducted from January 2015 to December 2015 in the Central Hospital of Wuhan. 400 patients who were clinical diagnosed thyroid cancer registered to this study. They were randomly assigned into two groups: LigaSure(L), Harmonic scalpel(H).Operative time drainage volume (day 1 after operation), haematoma, transient hypoparathyroidism, permanent hypoparathyroidism and nerve injury was recorded after operation. Results 353 of 400 patients underwent a total thyroidectomy and central neck dissection (level VI) for thyroid cancer. For L group, the mean operative time was longer than H group (72.62±12.71 minutes vs 69.79±12.36;P=0.044) . Transient hypoparathyroidism was significantly lower in the L group (8 vs 20 patients ; P=0.037). We found no significant differences between the L(n=173) and H(n=180) groups at drainage volume (40.72 ±15.64mL vs 40.42±16.49 mL;P=0.868) and transient voice damage (5 vs 8 patients ; P=0.45). No patients experienced permanent nerve injury or hypoparathyroidism. Conclusion Both Ligasure vessel sealing system and harmonic scalpel were safe in total thyroidectomy with a single instrument. The complications rate was similar among the two groups. Harmonic scalpel was faster in doing operation. Transient hypoparathyroidism appeared less in using LigaSure device.关键词
