Effect of 1, 25-Dihydroxy Vitamin D3 on the Inflammation Status in Diabetic Nephropathy

Abstract
Objective To investigate the effects of 1, 25-dihydroxy-vitamin D3 on the inflammation factors in diabetic nephropathy. Methods A total of 90 patients with type 2 diabetes mellitus were divided into the diabetes mellitus group ( group A, n=30 ) , early diabetic nephropathy group ( group B, n=30 ) , clinical diabetic nephropathy group ( group C, n=30 ) . DN patients were randomly divided into the treatment group receiving routine therapy plus 1, 25-dihydroxy-vitamin D3 and control group receiving routine therapy accord-ing to gender and age. All patients were observed for 24 weeks to compare the changes of 1, 25-dihydroxy-vitamin D3, CRP, IL-6, IL-17, TNF-α, MCP-1, ACR before and after therapy. Results The levels of 1, 25-dihydroxy-vitamin D3 in group A was the highest. Compared with group A, the levels of 1, 25-dihydroxy-vitamin D3 were lower in group B and group C ( P < 0. 05 ) , and espe-cially in group C with a statistically significant difference with group B ( P < 0. 05 ) . The levels of CRP, IL-6, IL-17, TNF-α, MCP-1 in group B and group C increased more obviously compared with group A ( P < 0. 05 ) , and the levels of CRP, IL-6, IL-17, TNF-α in group C increased more than group B ( P < 0. 05 ) . The level of 1, 25-dihydroxy-vitamin D3 increased and the levels of CRP, IL-17, TNF-α, ACR decreased in the treatment group, and the levels of CRP, IL-6, IL-17, TNF-α, MCP-1 showed no significant differences in the control group( P > 0. 05). The levels of IL-17, TNF-α and ACR in the treatment group(P < 0. 05)were signifi-cantly lower than those in the control group after treatment ( P < 0. 05 ) . The level of 1, 25dihydroxy-vitamin D3 was negatively corre-lated with the levels of CRP, IL-17, TNF-α( P < 0. 05). Conclusion There is lack of 1, 25-dihydroxy-vitamin D3 in patients with diabetic nephropathy, and the levels of inflammation factors are higher. The therapy of 1, 25-dihydroxy-vitamin D3 can reduce inflam-mation status in patients with diabetic nephropathy and decrease urinary protein.关键词
