Abstract
Objective To analyze the error causes of the drug dispensing in the outpatient pharmacy and to promote preventive mea-sures. Methods Retrospective analysis was applied to analyze the prescriptions in 2013,then the causes of 132 prescriptions with errors were investigated,and the improvement strategies were proposed. The errors in the outpatient pharmacy in 2014 and 2015 were summa-rized and compared with the ones in 2013. Results The major errors of the outpatient pharmacy drugs in 2013 were outpatient pharmacists errors,in which the drug quantity errors(23. 48%),drug dosage forms and specification errors(36. 36%) and errors caused by similar packaging or names(20. 45%)were the most common. Through the implementation of improvement measures,the drug error rates in the outpatient pharmacy in 2014 and 2015 were significantly reduced. Conclusion The analysis and discussion of outpatient pharmacy error causes and preventive measures can effectively ensure the quality improvement of drug dispensing in the outpatient pharmacy,provide patients with safer pharmacy services,ensure the smooth progress of medical work,which is significant in maintaining a good doctor-patient relationship.关键词
