不同部位游离静脉皮瓣修复手部软组织缺损的临床疗效对照研究
Clinical Efficacy of Control Study of Different Parts of Free Vein Skin Flap in Repairing Soft Tissue Defect in Hands

Abstract
Objective:Our retrospective study was aimed to explore and analyze the clinical efficacy of repair of soft tissue defects in hand with free vein flap of different parts.Method:A total of 100 patients with soft tissue defect treated in our hospital from March 2015 to October 2015 were selectd.They were divided into 4 groups based on flap donor area,namely group A,B, C and D,25 cases in each group.The patients in group A were rapaired with forearm forearm free vein flap.The patients in group B were rapaired with medial tarsal free vein flap. The patients in group C were rapaired with free venous flap of anterior region of wrist.The patients in group D were rapaired with dorsal free vein flap.The blood loss,infection rate,primary healing rate,skin flap survival rate and hand function recovery of the patients were observed,and the postoperative quality of life was followed up. Result:The average amount of bleeding during operation and the average amount of bleeding after operation of group B were significantly higher than those of group A,and the differences were statistically significant between two groups(t=25.67,P<0.05;t=13.25,P<0.05).At the same time,the average intraoperative blood loss of group B was significantly higher than that of group C and D,and the differences were statistically significant (t=12.79,P<0.05;t=10.36,P<0.05).The postoperative infection rate of group B and C were 0,which were significantly better than those of group A and D,and the differences were statistically significant(χ²=20.38, P<0.05;χ²=20.38,P<0.05).The highest healing rate of group B was 96.00%,which was significantly higher than those in group A,group C and group D,and the differences were statistically significant(χ²=6.72,P<0.05;χ²=7.61,P<0.05;χ²=6.29,P<0.05).At the same time,the survival rate of group B was 100%,which was significantly higher than those of group A,group C and group D,the differences were statistically significant(χ²=4.70,P<0.05;χ²=5.91,P<0.05;χ²=6.29,P<0.05).Patients in group B with a total of 20 cases had good hand function evaluation,accounting for 80.00%,wa significantly higher than those of group A,group C,group D, and the differences were statistically significant(χ²=10.21,P<0.05;χ²=11.87,P<0.05;χ²=11.02,P<0.05). At the same time,the patients in group B without a case of hand function evaluation was poor,the results were significantly better than those of group A,group C,group D,and the differences were statistically significant(χ²=20.38,P<0.05;χ²=30.01,P<0.05;χ²=20.38,P<0.05).The postoperative quality of life scores of group B were significantly higher than those of group A,group C and group D,and the differences were statistically significant(t=6.89,P<0.05;t=6.59,P<0.05;t=7.34,P<0.05).Conclusion:Free venous flap of medial tarsal dues to its physiological flap blood supply features in this study achieved good clinical efficacy,with the advantage of high flap survival rate, hand function recovery,which has important clinical significance in the repair of soft tissue defect in the hand.关键词
