Abstract
Objective To analyze the risk factors of common bile duct stones recurrence after choledocholi⁃thotomy. Methods Retrospective analysis was designed in 385 patients with common bile duct stones from Feb. 2009 to May 2012 in our institution, and chart review of four factors was performed (the status of general health , biliary tract, concretion and treatment). Results A recurrence developed in 58 of 385 postoperative patients (15.1%). Common bile duct stones recurrence rate of patients over 60 years old was significantly in⁃creased (P=0.009). In univariate analysis, common bile duct stones postoperative recurrence was significantly as⁃sociated with peripapillary diverticulum, common bile duct diameter≥ 10 mm, distal common bile duct stenosis, multiple calculi, and stone diameter≥ 10 mm(P=0.002,P=0.006,P=0.01, P=0.167, P=0.004 , P<0.001), how⁃ever , in patients with gallbladder stones and common bile duct stones the stone recurrence showed have no cor⁃relation (P=0.167). The recurrence rate in patients with multiple stones(≥70) showed 18.58%,while that was 6.78% in those with single stone. The recurrence rate increased marked(P=0.004). The recurrence rate in those with stones of diameter ≥10 mm showed 21.65%, while that was 12.85% in those with stones of diameter <10 mm. The difference showed statical differences (P=0.036). The laparoscopic exploration group was compared with the traditional surgery group, there was higher recurrence rate (P<0.001). Patients receiving Chinese crude drug treatment , the rate of recurrence was lower than the other group that receive none postoperative treatment. Conclusions Age, peripapillary diverticulum, common bile duct diameter≥ 10 mm and multiple calculi are the main risk factors for Common bile duct stones recurrence , whereas, traditional Chinese medicine treatment could decrease the recurrence rate.关键词
