降钙素原监测在ICU不同病原菌感染中的临床价值

Abstract
Objective To investigate the clinical value of serum procalcitonin ( PCT ) level in ICU patients with different pathogens Infection. Methods 380 cases with infections were selected in ICU from Aug 2013 to Dec 2015 in our hospital. The PCT levels were detected, meanwhile bacteria species were isolated and identified in duration of hospital stay. The clinical value of PCT level were identified for different pathogenic bacteria infection based on the receiver operating characteristic curve ( ROC ) . Results The PCT level with positive rate of G-bacteria was significantly higher than that with fungal and G+bacterial infection(all P<0. 01). However, there was no difference between fungal and G+bacterial infection(P>0 . 05 ) . The PCT level at 2 . 11 ng · mL-1 could distinguish G-bacteria infection from G+bacteria infection with sensitivity 82 . 62% and specificity 80 . 24%, and the area under curve ( AUC ) was 0 . 89 . The PCT level at 5. 08 ng·mL-1 could distinguish G-bacteria infection from fungal infection with sensitivity 68. 16%and specificity 73. 78%, and the AUC was 0. 78. Conclusion PCT level had a high value for identification of G- bacteria with fungal and G+bacteria infection, but low value for fungal and G+ bacteria identification.关键词
