肺炎支原体液体培养法的实验诊断学评价

Abstract
Objective To study the value of results of liquid culture test for mycoplasma pneumoniae( MP) by taking par-ticle agglutination test( PA) as the standard,and to explore the optimization of the liquid culture test for mycoplasma pneumoniae. Methods MP was detected in throat swabs of 2991 cases collected in our hospital from May 2014 to September 2015 and the re-sult was called liquid culture(LC)1. The LC with positive results was separated and cultured on plate. And the cultured bacteria were inoculated in MP liquid medium together with standard strain after identified,to determine the possibility of contamination. And LC2 was obtained by sieving false positive results. PA method was used to detect of the titer of serum mycoplasma pneumoniae in every patient. Three-dimensional chi-square test was also used to calculate sensitivity,specificity and accuracy of LC1 and LC2. The difference of the distribution of each age group and its interference with bacterial distribution was analyzed by applying Kruskal-Wallis rank-sum test. The false-positive results of MP liquid culture of specimen-isolated strain and homologous strain were compared by using t test. Results The sensitivity,specificity,accuracy,misdiagnosis rate,misdiagnosis rate and Youden index of LC1 and LC2 in the three-dimensional chi-square test were respectively 82. 2%、90. 2%、9. 8%、17. 8%、0. 724 and 82. 2%、96. 4%、3. 6%、17. 8%、0. 786. It can be seen that the specificity of LC2 was remarkably improved compared with LC1 ( P <0. 05). And LC1 had a higher misdiagnosis rather than LC2. There were significant differences in the distribution of interfering bac-teria between the age group of 12 years old and 13~50 years old and the age of 50 years old(P=0. 01). The lowest concentration of causing false positive results in MP liquid medium was lower than that of standard strain. Conclusion MP liquid culture(CL1) and improved liquid culture(CL2) both have a high clinical diagnostic value in the premise of eliminating interference of bacteria separated from oropharyngeal.关键词
