超声引导下经皮微波消融与射频消融治疗原发性肝癌的临床疗效对比分析

Abstract
Objective:To compare the clinical efficacy of microwave ablation and percutaneous radiofrequency ablation in the treatment of primary hepatic carcinoma.Methods: 91 patients with primary hepatocellular carcinoma (114 lesions) were divided into 48 cases of microwave ablation group (MWA) and 43 cases of radiofrequency ablation group (RFA) randomly, which were treated with radiofrequency ablation and microwave ablation, respectively. After treatment, the complete ablation rate, local tumor progression rate, and recurrence rate in 1 year and 2 years after surgery were compared and analyzed.Results: There was no significant difference between the two groups in total complete ablation rate and total local tumor progression rate (x2=0.0005,x2=2.0468;P>0.05). The complete ablation rate of MWA group in tumor 3-5cm was significantly higher than that of RFA group and the local tumor progression rate of MWA group with tumor 3-5cm were obviously lower than that of RFA group(x2=4.0368, P=0.0445). There was no significant difference between the two groups in complete ablation rate and local tumor progression rate with tumor <3 cm (x2=0.0076, P=0.9829;x2=1.3810,P=0.2399). There were no significant differences in the incidence of fever, pleural effusion and pain in liver area (fever,x2=0.2669,P=0.6054; pleural effusion,x2=0.0588, P=0.8084; pain in liver area,x2=0.4711,P=0.4925). There was no significant difference of the recurrence rate for 1 year and 2 years after surgery between the two groups (1year,x2=0.0381, P=0.8453; 2 years,x2=0.0148,P=0.9030). Conclusion:Both microwave ablation and radiofrequency ablation in the treatment of primary hepatocellular carcinoma shows a good clinical efficacy and safety. Radiofrequency ablation may have better therapeutic effect for smaller tumors, but microwave ablation may have some advantages with medium or large tumors.关键词
