Value of 3.0T MR high-resolution myocardial perfusion imaging in diagnosis of myocardial infarction

Abstract
ObjectiveTo explore the value of 3.0T MR High-resolution myocardial perfusion imaging (MRHMPI) in detecting the viable myocardium and infarcted myocardium.Methods 38 patients with history of myocardial infarction from June 2014 to May 2016were selected as the study objects. They were scaned with morphology, MR-cine and MRHMPI. 8 cases were selected to be treated with routine scanning and high resolution scanning, and the morphological and delayed images were compared.Results Hypoperfusion showed in 29 cases (76.3%) during the first-pass imaging, and normal perfusion in 9 cases(23.7%). Delayed hyperenhancement showed in 35 cases (92.1%) including 12 (35.0%) cases of transmural hyperenhancement, 9 (23.7%) cases of non-transmural hype-renhancement, 14 (44.7%) cases of mixed hyperenhancement and 3 (7.9%) cases of normal in delayed scan. 129 myocardial segments impaired in 38 cases totally 646 myocardial segments. 83 (64.3%) segments with systolic thickening ratio descended. The motion of myocardial wall decreased in 19 cases, disappeared in 8 cases and inconsistent motion in 5 cases. The image quality of 38 cases with High-resolution scan was signi-ficantly higher than that of the conventional scan in 8 cases, and the difference was statistically significant (P<0.01).Conclusion The position, scope and wall permeability of injured myocardium with signal changes can clearly be shown in MRHMPI, and the changes of wall thickness and motion of impairedmyocardium can be observed with MR-cine. Viable myocardium and infarcted myocardium can effectivelyevaluated in MRHMPI combined with MR-cine. It can provide objective evidence for clinical diagnosis, treatment and prognosis of myocardial infarction patients.关键词
