微创Carroll手术治疗先天性马蹄内翻足疗效初步分析
Preliminary analysis of efficacy of mini-invasive Carroll's technique in the treatment of congenital club-foot

Abstract
Objective To retrospectively explore the short-term efficacy of mini-invasive Carroll's tech-nique in the treatment of Dimeglio grades Ⅲ &Ⅳ congenital clubfoot (CCF). Methods From July 201 4 to January 201 6,a total of 27 Dimeglio gradesⅢ&ⅣCCF patients with 33 clubfeet were treated.There were 1 8 males (22 feet)and 9 females (1 1 feet).The age range was 3 months to 3 years.CCF was treated by mini-in-vasive Carroll's technique via three small incisions in heel,medial side of foot and planta pedis respectively. Talocalcaneal and talonavicular joints were subject to manipulative reduction.Then long-leg plaster cast fixation was applied for 6~8 weeks.At 1 week post-operation,operative wounds received dressing changes.Re-exami-nations were performed at 1 ,2 and 3 months.The follow-up period was every 6 months.According to the Dimeglio score,Pirani score,radiography and function,the recovery of clubfeet was evaluated. Results After satisfactory releases,the deformities of heel,medial side of foot and planta pedis were fully corrected.The lengths of incisions were 0.5 ~1 .0 cm.During a follow-up period of 3 ~1 8 months,the functions of treated clubfeet recovered satisfactorily.The efficacies were evaluated by Dimeglio score,Pirani score,radiography and ankle joint function:excellent (75.76%),25 clubfeet of 20 patients;good (1 8.1 8%),6 clubfeet of 5;decent (6.06%),2 clubfeet of 2.The good or excellent rate was 93.94%.And no complications occurred. Scars were not obvious. Conclusions For Dimeglio grades III &IV congenital clubfoot,mini-invasive Carroll 's technique can sufficiently correct the deformity with fewer complications and excellent wound healing.关键词
