慢性心力衰竭合并肾功能不全患者临床特征的回顾性研究
Retrospective study of clinical characteristics of chronic heart failure combined renal insufficiency

Abstract
Objective To investigate the clinical characteristics of chronic heart failure complicated with renal insufficiency.Methods A retrospective analysis of 374 patients with chronic heart failure as the first diagnosis of hospitalized patients,according to the modified MDRD equation of the patients with esti-mated glomerular filtration rate,all the patients were divided into control group (n =106),mild renal in-sufficiency group (n =129)and renal insufficiency group (n =139).The clinical characteristics and prognosis were compared among groups.Results Renal function of chronic heart failure patients deterio-rated with age,and the average hospital stay was significantly prolonged for renal insufficiency group. The red blood cells and the hemoglobin of the renal insufficiency group were significantly lower than the mild renal insufficiency group and normal group (P <0.05).For renal insufficiency group,the potassium concentration,serum phosphorus concentration,serum uric acid concentration,urea nitrogen concentra-tion,brain natriuretic peptide (BNP)and troponin T (TNT)positive rate,and the dead cases of renal inefficiency in two years were higher than mild renal insufficiency group and normal group (P <0.05). Age,red blood cell,hemoglobin,creatinine,urea nitrogen concentration and TNT were the risk factors of death in two years.Conclusion Chronic heart failure complicated with renal insufficiency has a high incidence in aged people,and the hospital stay is relatively long.The mortality increases in two years. The patients with chronic heart failure combined renal insufficiency has low level of red blood cells and hemoglobin,as well as the high level of serum potassium concentration,blood phosphorus concentration, blood urea nitrogen,blood uric acid,brain natriuretic peptide and troponin T.关键词
