Abstract
OBJECTIVE:To investigate the intervention on irrational medical orders by prescription audit pharmacists in the Pharmacy Intravenous Admixture Service ( PIVAS ) of the First Affiliated Hospital of Kunming Medical University ( hereinafter referred to as “our hospital”) , so as to provide reference for the rational application of drugs in clinic .METHODS:Analysis was conducted on the irrational medical orders intervened by prescription audit pharmacists in PIVAS and the improvement suggestions provided by clinicians from Apr .2015 to Mar.2016. RESULTS:Totally 3 645 cases of irrational medical orders for pharmacists intervention were analyzed , and 3 275 cases were changed by clinicians with the rate of 89.85%.The irrational types of drug application were respectively overdose administration , improper concentration , improper dosing frequency , improper solvent , input error , irrational drug selection , improper compatibility , over course of treatment , repeated administration , improper route of administration , over indications of drug application and other irrational medications .CONCLUSIONS: The application rationality of drug application can be guaranteed greatly through the intervention of on total medical orders review by prescription audit pharmacists , which promote the rational application of drugs in clinic and benign development of PIVAS .关键词
