HPV E6/E7基因和HPV L1基因DNA检测的比较及意义
Comparison and Significance of the DNA Detection of E6/E7 Gene and L1 Gene in HPV

Abstract
Objective:To compare the situation of the HPV16 subtype detection of the early gene(E6/E7) and the later gene(L1) in cervical tissue and analyze the significance in evaluating cervical cancer and cervical precancerosis.Method:The E6/E7 gene and the L1 gene of high risk HPV16 subtype were chosen and designed specific primers.Then PCR conditions were optimized and the new PCR detective method of high-risk HPV16 was established.76 cases of cervical tissues established were collected,which including 22 cases of chronic cervicitis and 54 cases of cervical intraepithelial neoplasias(CIN) and crervical squamous carcinoma.According to pathological diagnosis results,they were divided into two groups including control group(chronic cervicitis and CINⅠ) and high-grade cervical lesion group(CINⅡ,CINⅢ,crervical squamous carcinoma),and DNA was extracted and detected by new established method of E6/E7 gene PCR and HPV-L1 gene PCR.Then the detecting result was compared with clinical detecting method of HCⅡ.Result:A sensitive and specific PCR detective method based on E6/E7 gene and L1 gene for high-risk HPV16 type were established successfully.The positive rate of HPV E6/E7 PCR method of tissues was 55.3%(42/76),the positive rates were both 25.0%(19/76) with HPV L1 gene PCR method and HPV L1 HCⅡ method.The positive rate of HPV E6/E7 PCR method was obviously higher than other methods,and the difference was statistically significant(P<0.01).But the positive rates between HPV L1 gene PCR method and HPV- L1 HCⅡmethod was not statistically significant(P>0.05).The positive rate of HPV E6/E7,HPV L1 and HPV- L1 HCⅡ in the control group was respectively 29.0%,9.7% and 12.9%,which were respectively obviously lower than the positive rate of 73.3%,35.6% and 33.3% in high-grade cervical lesion group(P<0.05).The detection result about relevance between the early gene(E6/E7) and the late gene(L1) was analyzed,the result showed that HPV E6/E7 and HPV L1 were both related in the control group(P<0.05) and in high-grade cervical lesion group(P<0.05),but the others were not related(P>0.05).Conclusion:The E6/E7 gene detection is more sensitive,more efficient,more pragmatic than L1 gene.Combining the positive detection results of HPV16 E6/E7 gene with the pathological diagnosis above CINⅡclass can be one of practical methods of predicting and evaluating the early clinical cervical lesion and cervical cancer.关键词
