肝切除术和经肝动脉化疗栓塞术治疗晚期肝细胞癌的疗效对比
Comparison of efficacy between hepatic resection and transarterial chemoembolization for the treatment of advanced hepatocellular carcinoma

Abstract
Objective To compare the efficacy between hepatic resection ( HR) and transarterial chemoembolization ( TACE) for the treatment of advanced hepatocellular carcinoma (HCC).Methods Clinical data of 436 patients with advanced HCC were retrospectively analyzed,including 332 patients undergoing HR(HR group) and 104 patients undergoing TACE(TACE group).Homogeneity of baseline data between two groups was matched using propensity score analysis .The incidence of complications , mortality and survival were compared between the two groups.Results After propensity score matching ,204 cases and 102 cases were enrolled in the HR group and TACE group , respectively,there were no significant differences in the baseline data between the two groups (P>0.05) and comparability was good.After propensity score matching ,there were no significant differences in the incident rate of complications or mortality between the two groups (P>0.05),the median survival time of the HR group was 17.0 months,and was longer than that of the TACE group(11.7 months,P<0.05). Overall survival rates at 1,3,5,and 7 years after surgery of the HR group were higher than those of the TACE group (58.8%vs.48.7%, 22.3%vs.14.3%,18.1%vs.12.3%and 18.1%vs.6.8%,respctively).Conclusion The efficacy of HR is superior to TACE for the patients with advanced HCC .关键词
