GP73、AFP-L3、GPC-3和DCP联合检测在低浓度A FP小肝癌诊断中的价值及验证
Value and verification of combined tests of serum GP73,AFP-L3,GPC-3 and DCP in diagnosis of low concentration AFP of small primary hepatocellular carcinoma

Abstract
Objective To investigate the value of combined tests of serum Golgi protein 73 (GP73) ,lens culinaris agglutinin‐re‐active alpha‐fetoprotein (AFP‐L3) ,glypican 3(GPC‐3) and des‐gamma‐carboxy prothrombin(DCP) in the diagnosis of small prima‐ry hepatocellular carcinoma (PHC) with low concentration alpha‐fetoprotein (AFP) .Methods Fifty cases of small PHC with low concentration AFP ,60 cases of non‐PHC digestive system disorders and 40 healthy controls were collected .Serum GP73 ,GPC‐3 and DCP levels in each group were detected by using the enzyme‐1inked immunosorbent assay(ELISA) .AFP‐L3 was separated by mi‐crospincolumn ;the levels of AFP‐L3 in separated eluent and total AFP were quantified by electrochemiluminescence immunoassays ;the percentage of AFP‐L3 to total AFP was calculated .Results The levels of GP73 ,AFP‐L3 ,GPC‐3 and DCP had statistically sig‐nificant difference among three groups (P<0 .01) .Positive detection rates of four biomarkers in the PHC group were the higher than those in other groups (P< 0 .01) .Among single index detection ,the sensitivity ,accuracy rate and area under ROC curve of GP73 for diagnosing small PHC were the highest ,which were 72 .0% ,86 .7% and 0 .826 ;the specificity of GPC‐3 was the highest and was 98 .0% .The sensitivity ,specificity ,accuracy rate and area under ROC curve of 4‐marker combined detection were 78 .0% , 94 .0% ,88 .7% and 0 .867 respectively .Conclusion The single detection of GP73 or AFP‐L3 has higher diagnostic value for small PHC with low concentration AFP .The combined detection of multiple biomarkers can improve the diagnostic value and is used in the assisted diagnosis of low concentration AFP of small PHC .关键词
