Abstract

Objective To investigate key points of diagnosis and differential diagnosis,and misdiagnosed causes of nonischemic central retinal vein occlusion( CRVO),and to propose the possible measures to avoid misdiagnosis. Methods Clinical data of 31 misdiagnosed patients with nonischemic CRVO during March 2010 and 2015 was retrospectively analyzed. Results Among the patients,9 patients were misdiagnosed as having macular edema,6 patients as having dry age-related macular degeneration,6 patients as having ocular ischemic syndrome,5 patients as having diabetic retinopathy,3 patients as having anterior ischemic optic neuropathy and 2 patients as having optic disk edema. Misdiagnosed time was 1 week to 3 years. All the 31 patients were misdiagnosed for 34 times,8 times occurred in three-level hospitals,and 26 cases occurred in two-level hospitals. All the patients were confirmed the diagnosis of nonischemic CRVO by comprehensive analysis after overall eye examinations such as visual acuity,intra-ocular tension,anterior chamber angle,pupillary light reflex,fundus fluorescein angiography,visual field and optical coherence tomography,skull imaging information( CT or MRI)and related laboratory ex-aminations. Conclusion Clinical manifestations of nonischemic CRVO lack specificity,and therefore differential diagnosis is difficult,and it is easily misdiagnosed. Improving the ability of comprehensive analysis and differential diagnosis are the key points to avoid misdiagnosis and mistreatment.
